Without OCR: the document's own text, restructured
A born-digital PDF carries its exact text; conversion's work is rebuilding Word's structures — paragraphs, styles, tables — from positioned print. Accuracy of the words: exact. Fidelity of the layout: depends on the document's complexity, mapped honestly in what converts well.
With OCR: recognised text, then restructured
A scan converts to pictures-in-a-document unless recognition runs first. The chain adds one property: the text is as good as recognition was — excellent on clean scans, flagged per page by confidence where it wasn't. Editing responsibility shifts accordingly: proof the low-confidence pages against the image before trusting the prose.
Check the intent, too
Half of "I need it in Word" cases actually need something narrower: quotes and paragraphs (that's extraction), a table (that's PDF to Excel), or one correction (that's the editor, no round-trip at all). The narrower tool is always the better result.
Troubleshooting
The .docx contains page images. You converted a scan without OCR — run the chain in order. Recognised text has errors in one section. Check that section's confidence and the scan's quality there; rescan if it was the source. Layout exploded in Word. Designed pages do that — edit content, not layout, or return to the design source.
